Update 1-6-22:

MOUNTAIN VISTA CURRENTLY HAS 3 RESIDENTS AND 5 STAFF MEMBERS WHO HAVE TESTED POSITIVE FOR COVID-19. AS A RESULT THE HEALTH CENTER IS CONSIDERED TO BE IN AN OUTBREAK. IF YOUR LOVED ONE IS POSITIVE AND YOU ARE THE POWER OF ATTORNEY YOU WILL BE NOTIFIED.  WE WILL KEEP YOU UPDATED AS CIRCUMSTANCES CHANGE.

All Visitors, you have the potential to be exposed to SARS-CoV-2 in the Facility.

 

Indoor visitation during an outbreak response

Resident and visitor should wear well-fitting source control facial coverings (if tolerated) and physically distance.

Facial coverings and physical distancing recommendations should also be followed for vaccinated residents.

Outdoor visitation is allowed, but residents should wear well-fitting Facial coverings (if tolerated), maintain physical distancing from others, and not linger in common spaces when moving from their rooms to the outdoors

Visitors should physically distance themselves from others while in the facility.

Visitors should visit with residents in single-person rooms; or designated visitation areas when only the resident they are visiting with is present.

Visitors should physically distance themselves from the resident they are visiting unless the resident and all of their visitors are fully vaccinated, then they can choose to have physical contact with one another.

 Visitation While Infectious

For the safety of visitors we encourage residents and families to limit in-person visitation.

Visitors may only visit the resident room and minimize their time spent in other locations in the facility.

Pets other than ADA service animals should not be included in communal dining or group activities.

We recommend that children, who are able, to consistently wear a mask and social distance while in the facility.

 

Number of Visitors Allowed Per Visit

Although there is no limit on the number of visitors a resident may have at one time, visits should be conducted in a manner that adheres to the core principles of COVID-19 infection prevention and does not increase the risk to other residents. The CDPHE recommends allowing no more than two visitors per resident per room.

We ensure physical distancing be maintained during peak visitation and void large gatherings.
