Update 12-18-20

As of December 15, 2020, we have one new COVID-19 positive case. The two residents that previously tested positive for COVID 19 have recovered. We continue to test residents and staff twice a week due to outbreak status and the 2-week county positivity rate. If your loved one tests positive, we will notify you individually.

Our residents and staff safety continues to be our top priority. We will do everything we can to keep COVID-19 out of our building and are following all Colorado State Health Department and CDC guidelines.

We continue to screen all staff and follow stringent infection control practices. All residents who reside in the health center are assessed three times a day for an elevated temperature and COVID-19 symptoms. All residents who have tested positive since November have been asymptomatic, therefore we continue to encourage residents to wear masks and maintain a distance of 6 ft. from one another. We have also asked our residents to limit their movement throughout the building to help reduce the spread.

Update 10-30-20

Due to the increased number of positive COVID-19 cases in Jefferson County and guideline set by Colorado Department of Public Health and Environment (CDPHE) we are required to temporarily halt our indoor visitation. However, we will continue our outdoor and window visitation practice, weather permitting.

Visitation Schedule

Outdoor visitations will take place in front of our building. You can schedule your appointment daily between 9 am – 11 am and 1 pm – 4 pm. On Thursdays appointment can be scheduled from 4 pm -7 pm. All appointment will last 30 minutes. Children under the age of 18 years of age are welcomed and up to eight (8) visitors at a time outdoors. Our staff will clean & sanitize between each visit to protect all residents and visitors.

Scheduling a Visit

To schedule a visit please call our front desk at (303) 421-4161. If weather prevents outdoor visitation we encourage you to consider a Zoom, Skype, FaceTime call or Facebook Chat. If you need help setting up your visit using one of these methods please contact the community and we will be happy to help.

Visitation Screening

You will be screened prior to your visit:

  • Visitors will have their temperature taken
  • Visitors must complete a health screening form
  • If you do not pass any portion of the screening process, your visit will be declined
  • No food or drinks allowed during visitation

During Your Visit

There will be no physical contact with your loved one. If there is contact, we will end your visit and potentially discontinue future visits. Residents who are on isolation will not participate in any visits until they meet the criteria and are allowed to come off of isolation.

Update 10-27-20

We are happy to announce that we are preparing for indoor visits! Due to our ever changing Colorado weather, we have been preparing our policies since Governor Polis announced our “Protect Our Neighbors” phase, which allows for indoor visit. Please remember as the visitor that even one case of COVID-19 introduced to our community will immediately shut down indoor visitation. So, it is of utmost importance that we all remain diligent about the policies.

Visitation Times Visits will take place in our main lobby between 9am-11am and 1pm-4pm daily and 4pm-7pm on Thursdays, and will last 30 minutes.

Screening:  The screening process includes:

· Having your temperature taken
· Filling out and signing the screening forms
· Proof of a negative Covid-19 test taken within 48 hours prior to your visit (Please provide us with a copy of your negative results for our records)
· If you do not pass any portion of the screening process, your visit will be declined

Masks during indoor visits: you must wear a mask covering your nose and mouth at all times. No food or drinks allowed during visitation.

Visits to ensure our residents’ safety: there will be a barrier between you and your loved one during indoor visits. No more than 2 visitors at a time. All visitors must be 18 years old or older. There will be no physical contact with your loved one. If there is contact, we will end your visit and potentially discontinue future visits. Residents who are on isolation will not participate in indoor visits until they meet the criteria and come off isolation.

Our staff will clean & sanitize between each visit to protect all residents and visitors.

Making an Appointment To schedule a visit, please call our front desk at (303) 421-4161. If weather permits, you can always bypass the COVID-19 test and schedule an outdoor visit, or consider a FaceTime call or window visit. Children under the age of 18 years of age are welcomed and up to eight (8) visitors at a time outdoors.

Update 10-6-20

At Mountain Vista we currently have one positive COVID-19 case in the Health Care Center. This resident is free of COVID-19 symptoms and will come out of isolation and return to their room on 10/8/2020. To maintain the health and safety of everyone on the campus we will continue to follow the guidelines and recommendations set forth by the CDC and State Health Dept.

Staying in Touch

While visitation restrictions remain in place until October 8th, we encourage you to keep in contact with loved ones using digital and social media channels such as Zoom, Facetime, Skype and of course phone calls. Please contact our community and one of our staff members will be happy to help you.

Visitation

Even with the single positive COVID-19 case window visits will remain in place. To help better prepare you for your visit we have outlined the policies for window visits below.

Window Visitation Policy

POLICY STATEMENT

Our facility permits residents to receive visitors subject to the resident’s wishes and the protection of the rights of other residents in the facility. While COVID-19 continues to present a substantially increased risk of mortality among older adults and individuals with underlying medical conditions in the State of Colorado, isolation of individuals in nursing homes, group homes, assisted living communities, and other congregate settings impose substantial physical and mental health consequences for these residents. Social distancing and use of electronic means of communication remain strongly recommended; however, the State of Colorado, in an effort to promote the health and well-being of Colorado’s residents, will allow for visitation in facilities under the following policy guidance.

 

POLICY INTERPRETATION and IMPLEMENTATION

Pre-Visit

Visitors should call ahead to arrange or schedule a visit. Prior to arrival for the scheduled meeting, the visitor should be provided information on the terms of the visit through a means provided by the facility (web, email, etc.). The information must contain:

  1. Information on COVID-19 and how the spread of the virus is minimized.
  2. Instructions for self-screening on the day of the visit along with information as to when the results of the screening would require a cancellation of the visit.
  3. Instructions for social distancing, requirements for wearing a mask, and keeping the window closed at all time.
  4. Instructions on where to arrive and wait at the facility for staff accompaniment to the designated meeting area. This information must include a notice that the visitor will be restricted from entrance into the building and may not leave the designated visiting area to interact with any other residents or staff.

 

Upon Arrival

The visitor must be greeted outside at a designated area by facility staff, and the staff member will:

  1. Perform temperature check and symptom screening in accordance with current CDC guidelines. Visitors with symptoms in the previous 14 days should not be allowed visitation. (Additional Guidance)
  2. Ensure the visitor has a face mask or cloth covering, and is wearing it appropriately.
  3. Collect and verify name and contact information of the visitor for the purposes of contact tracing.
  4. Remind the visitor not to open the window at any time.
  5. Escort the visitor to their loved one’s window.
  6. Escort the visitor out of the designated visiting area at the conclusion of the visit through the same path they arrived.
  7. The resident must have been screened on the day of the scheduled visit, prior to the visit occurring.

Window Area During Visitation

  1. As a part of the facility’s existing resident outdoor area, the separate designated meeting area should be isolated. The facility should ensure that residents not participating in visits, continue to have access to separate outdoor space.
  2. The designated outdoor area must be monitored to ensure it remains separated from the facility population and from facility staff.
  3. The number of visitors should be determined by using the outdoor social distance calculator, which will provide the allowable number based on the square footage of the area that will be designated for outside visits. The allowable number of persons (resident, staff, and visitors) is either the calculated number or 8, whichever is smaller.  Any codes, regulations, or ordinances requiring a smaller number of people must be followed. This number of maximum visitors allowed must be documented in the visitation plan.
  4. Furniture used for external visits should be appropriately disinfected between visits.
  5. The visitor must wear a face mask or covering for the duration of the visit. All staff and the resident must wear a surgical or cloth mask unless medically contraindicated.

Inquiries concerning visitation and access to the facility should be referred to the Administrator or his/her designee.

Restrictions

Facilities may NOT offer or allow outdoor visitation on the premises if:

  1. The resident has symptoms of COVID-19.
  2. The resident is in isolation or quarantine.
  3. The facility has an active outbreak.
  4. The facility with a history of a recent COVID-19 case or outbreak has not completed the required isolation period of 14 days.
  5. The county or the city where the facility is located is under the Stay-at-Home orders related to COVID-19.
  6. Statewide restrictions are implemented due increased cases of COVID-19.

Dear Residents and Family Members:

At this time we are happy to report that Mountain Vista is COVID free!  The State of Colorado has given us approval to start social visits for residents in the Assisted Living and we are truly humbled to help reunite you with your loved ones.

In the interest of keeping residents and visitors safe, all visitors must follow the guideline stated below and sign a form that states they have read, understand and will adhere to the guidelines. If a visitor does not follow the guidelines they may be restricted from future visits.

  • All visits must be scheduled prior to visit.
  • Visits are limited to 45 minutes and can be cancelled at any time.
  • Visits must take place within the Visitation POD
  • All visitors/residents must wear a mask
  • Visitor must remain on their own side of the POD
  • There can be no contact between visitors and residents
  • There must be a distance of at least 6 feet between visitors and residents
  • No more than 8 visitors at any one time
  • Each visitor must complete and pass a health screen at each visit. If you do not pass the screening you will be asked to leave the campus immediately
  • Parking for visitation is across from the Visitation POD and visitor will be escorted to the visitation area
  • No personal items can be left for or given to residents during a visit. All items must be dropped off at the front desk.

 It is important to know that outdoor visitation will not be allowed if a resident or the Mountain Vista Campus experiences another outbreak of COVID, therefore it is vital that everyone follow the visitation guidelines. If you are not feeling well or have any signs or symptoms of COVID please to do attempt to visit the Community.

 Assisted Living Visitation

Visits will begin Wednesday July 22nd. Assisted Living will schedule future visits on Saturdays, Mondays and Wednesdays from 8am-3pm. To Schedule your visit call-303-420-2192.

Health Care Center Visitation

In the first part of August we hope the state will approve visits for residents in the Mountain Vista Health Center. However, until that time we still have our visitation restrictions in place for the Mountain Vista Health Center.

Update 7-10-20:

At this time we are happy to report we have one resident at the Health Center finishing isolation for positive COVID test and happy to report there are no signs or symptoms and is doing well. We finished this week with no residents testing positive.

At the Assisted Living we continue to have no one positive or on isolation. If a resident is suspected or diagnosed with COVID-19 and we have permission to contact you, we will.

Summary of Guidance for outdoor visitation:

  • Must be Covid free for 28 days this includes signs and symptoms not necessarily positive tests.
  • CDPHE must give clearance to do outside visits and no orders from the state for restrictions.
  • Must schedule your visit.
  • Visitor must be greeted outside at designated area (rehab entrance by the canopy).
  • Must go through screening process-temperature taken, form filled out with contact information and receive COVID information
  • Must wear mask.

We anticipate the Assisted Living to have visits starting July 15 if everything goes well.

Update 7-2-20:

Mountain Vista Health Center

Currently two people in the Health Center have tested COVID-19 positive. These two individuals do not exhibit any symptoms however they will continue to be closely monitored for any changes in their health. We are also working with the Colorado Department of Health to develop guidelines regarding outdoor visitation. After we have more details we will post on this website.

Mountain Vista Assisted Living

In the Assisted Living area of the Community we do not have any residents that have tested positive for COVID -19.  We will continue to work diligently to keep residents and staff safe and healthy.

We will continue to follow the CDC recommendations regarding Personal Protective Equipment, sanitation procedures and visitation restrictions.

Update 6-25-20:

I personally want to thank all of you for your overwhelming support during this most difficult time. We have received many notes and phone calls of encouragement which kept us coming back to work each and every day. We have also received so many lovely donations such as supplies, food and financial support. Words cannot express how you made a difficult time better for those here at Mountain Vista. We are lucky to have such amazing residents and family members.

At this time we are happy to report there are only three residents in the Health Center who have tested positive for COVID-19 and are nearing the end of their isolation period. These residents have not exhibited any signs of the virus and are doing well. In Assisted Living we are clear of the COVID-19, and we do not have any residents who are being isolated. If a resident is suspected or diagnosed with COVID-19 and we have permission to contact you, we will do so.

We understand that you are concerned about your loved one, but it is crucial that we continue to restrict visitation to reduce the spread of this virus and continue to follow all CDC recommendations. If you would like an update on the health of your loved one we encourage you to call the Mountain Vista Health Center or our Assisted Living. Update 6-17-20:

We want to thank all the residents, families and staff for their continued effort during this time of uncertainty. The precautions you have taken to protect yourself and others has been effective and Mountain Vista is proud to say we have experienced a dramatic decrease in the number of COVID-19 cases, and for that we are thankful.

We anticipate receiving additional guidance from our state and federal government agencies as we learn more about COVID-19, and how we can best protect residents, families and staff.

Update 5-27-20:

During the past week, as businesses begin to re-open across the state and restrictions are relaxed, we are encouraged to a see aspects of life return to normal. However for us in healthcare, particularly senior living, it will take some time before we see a full return to “business as usual”. As a result we must continue to adhere to federal and state guidelines that restrict us from relaxing rules for visiting the community and other group activities at this time.

For residents, the staff is working hard to keep them engaged and enhance their lives through new and innovative activities that are mindful of social distancing and other requirements. Hallway bingo, doorway singing and virtual museum tours are just a few of the ways that we are supporting and engaging our residents. Our community has received puzzles, crossword, word search or adult coloring books and flowers. We are so grateful to families and friends of Mountain Vista for their generosity in supporting our residents!

 Resident COVID-19 Information

We are currently monitoring twenty seven COVID-19 positive residents. To date, fifty-five residents have tested positive at Mountain Vista Health Center. This includes the twenty seven residents currently being monitored. At Mountain Vista Assisted Living there is only one resident that has tested positive and they have been isolated following CDC guidelines. Until residents recover, they will be under constant observation noting any changes in their condition and we will continue to work closely with our medical director, clinical team and the state health department.

Over the past week Colorado Government Officials have taken initial steps to re-open business and establishments throughout the State. Like you, Mountain Visa is anxious for a return to normalcy where visitors are warmly welcomed into our Community and residents, families and friends can interact without restrictions.

However, because seniors are considered high-risk for COVID-19 infection, federal and state guidelines restrict us from relaxing rules for visiting and other group activities at this time.  Please understand this is a difficult time for all concerned and we will continue to do all we can to keep our residents and families connected in a way that is safe for everyone.

To ensure the health of residents and staff Mountain Vista initiated the testing of all residents and staff, which was completed on 5/18/20. Moving forward we will continue the testing process and update our website with new information.

Covid-19 (Coronavirus) Questions & Answers:

American Baptist Homes of the Midwest (ABHM) understands the concern family members have for their loved ones. The COVID-19 outbreak only heightens that concern. Hopefully, the answers to the questions below give you peace of mind that ABHM is doing everything possible to keep your family member safe.

How is ABHM monitoring and keeping residents from being exposed to COVID-19?

  • ABHM is in contact with our state Health Departments and Centers of Disease Control (CDC).
  • Centers of Medicare and Medicaid (CMS) sends out communications and updated information.
  • The Medical Director and Medical Team verify our current medical treatment plans are aligned with current clinical guidance and standards of practice.
  • Transportation is only provided for essential doctor appointments.
  • Social gatherings have been discontinued until further notice.
  • Proper Hygiene and social distancing is being taught to and implemented by our members, residents, employees and vendors.
  • Deep cleaning schedules have increased at all levels of care at ABHM Communities.
  • All employees and essential visitors/vendors are screened prior to entering our Communities.
  • Communal dining has been discontinued until further notice.

What are the signs and symptoms COVID-19 and will my loved one go to the hospital if diagnosed?

  • The signs and symptoms are similar to the A/B Flu: difficulty breathing, cough, fever (100 degree F), a general feeling of being sick or having no energy, or an uncomfortable feeling that something is wrong (malaise).
  • We will place the positive COVID-19 loved one (pending or positive test results) in isolation to prevent spreading it to other residents.
  • ABHM will care for your loved one as long as their symptoms remain mild and their other medical conditions do not flare-up, which could complicate the recovery process.
  • If symptoms do become severe and require more hours of nursing and medical team attention, the resident may be transferred to the hospital.

What happens if an ABHM employee gets COVID-19?

  • ABHM is following all CDC recommendations related to employee and COVID-19.Unfortunately if one of our employee does become ill with COVID-19 that employee will need to be quarantined at home and recover under the care of their physician.  Once recovered that employee will be screened prior to coming back to work to assure that he/she is ready to return to work per the CDC guidance and approval of their physician.

Is there COVID-19 test kits readily available at ABHM communities?

  • At this time test kits are not available at ABHM communities. The local health department and the primary care physicians will decide who should be tested based on the symptoms of the resident.
  • The health department will provided the test kits or instruct a private lab to deliver the test kit to the community.

Are there concerns with medical/cleaning, food supplies or deliveries?

  • On a daily basis ABHM Teams consisting of the Administrator, Director of Nursing and Infection preventionist discuss and review food service, supplies and all other community needs to ensure we have ample supply to serve residents .
  • There have been reported shortage of personnel protection wear (PPE). However at this time ABHM is experiencing routine deliveries and is in continuous contact with suppliers.

Does the CDC recommend the use of a facemask to prevent COVID-19?

  • CDC does not recommend that people who are healthy wear a facemask to protect themselves from respiratory illnesses, including COVID-19.
  • You should only wear a mask if a healthcare professional recommends it.
  • A facemask should be used by people who have COVID-19 and are showing symptoms.
  • The use of facemasks is crucial for health workers and other people caring for someone infected with COVID-19 in close settings.

Dear ABHM Residents and Families:

Although there is no presence of the coronavirus (COVID-19) in our community, Mountain Vista Senior Living Community is taking a proactive approach to mitigate the risk of spreading COVID-19.

Age is a risk factor for COVID-19, so that makes the population at our communities vulnerable to this illness. Underlying medical conditions can also increase the risk and severity of the sickness so we encourage residents to assess your individual situation and limit your exposure as you see fit.

In addition to the recommendations from the Center for Disease Control (CDC) and in an effort to keep people safe, healthy and prevent the spread of all communicable illnesses, Mountain Vista has adopted a “no visitor policy” as follows:

  • All doors will remain locked at all times.
  • All vendors and essential healthcare personnel will have to enter through the main entrance and will have to answer the questions recommended by the CDC.
  • Exceptions will be made for families during major change in condition of their loved one and/or during end of life process.
  • Any essential visitor who has traveled to China, Iran, South Korea, Italy, Japan, Hong Kong, Austria, Belgium, Czech Republic, Denmark, Estonia, Finland, France, Germany, Greece, Hungary, Iceland, Italy, Latvia, Liechtenstein, Lithuania, Luxembourg, Malta, Netherlands, Norway, Poland, Portugal, Slovakia, Slovenia, Spain, Sweden, Switzerland, Monaco, San Marino, Vatican City, in the last 14 days must stay out of the building and self-quarantine for at least 14 days.
  • Any essential visitor who has traveled to Seattle, Washington, New York, California, Singapore, Thailand or Taiwan must monitor yourself for symptoms and wear a mask while at our community.
  • Any essential visitor with signs or symptoms of respiratory infection may not enter the building.

Mountain Vista will be cancelling all non-essential appointments at this time.

The facility will be available to answer any questions and address any concerns. Facetime, Skype and telephones will be available for you to keep in touch with your loved ones while we are going through this evolving situation. We will continue to provide updates when appropriate.

Common sense approaches we use to combat influenza will be some of the most effective prevention methods for COVID-19. The safety of the residents and staff remain our top priority.
